Shin Splints: What They Are, What Helps, and When It's Something Worse

Shin splints are the classic autumn injury. Training ramps up for an autumn race, or you get back into running after a patchy summer, and within a few weeks there's an aching soreness along the inside of the shin that's fine at rest, sore at the start of a run, and grumbling for hours afterwards. It's one of the most common running complaints there is, it's usually very manageable, and it has one important lookalike that isn't. Here's how to handle it properly.

What Shin Splints Actually Are

The clinical name is medial tibial stress syndrome, which is more descriptive than it sounds. Pain develops along the inner edge of the shinbone, typically over the lower two thirds, where the muscles of the lower leg attach to the bone. When the load going through that area exceeds what the bone and surrounding tissue can currently tolerate, it becomes irritated and inflamed.

The important word there is load. Shin splints are not an injury in the sense of something tearing, they're a capacity problem: you've asked the tissue to do more than it's currently built for. That framing matters, because it tells you the solution is to reduce the demand temporarily and then build the capacity, rather than to rest until it stops hurting and go straight back to where you were.

Runners get it most, but so do dancers, footballers, court sport players, and anyone who has suddenly taken up a lot of walking or standing.

Why It Happens

Almost always, something changed. The classic trigger is a rapid increase in mileage or intensity, the too much, too soon problem, and the rough guideline of increasing weekly volume by no more than around ten percent exists precisely because of injuries like this one.

Other common contributors: switching to harder surfaces, particularly moving from trails or treadmill to pavement; running shoes that have quietly done several hundred miles and lost their cushioning; weak calves and hips that leave the shin absorbing more than its share; and running mechanics that involve overstriding, landing heavily with the foot well ahead of the body.

If your shins started complaining within a few weeks of any of those changes, you've almost certainly found your cause.

Shin Splints or Stress Fracture? The Distinction That Matters

This is the most important section of this article. A tibial stress fracture can start out feeling like shin splints, and it needs completely different handling, so it's worth knowing how they differ.

Shin splints: pain spread diffusely along a stretch of the shin, several centimetres or more. Often sore at the start of a run and eases somewhat as you warm up. Tender over a broad area when you press along the bone. Settles reasonably quickly with rest.

Possible stress fracture: pain concentrated at a single specific point you can cover with a fingertip. Tends to get worse as the run goes on rather than easing. May ache at rest or wake you at night. Sometimes accompanied by a small area of swelling. Hopping on that leg is typically sharply painful.

If your symptoms fit the second description, stop running and get it assessed. Stress fractures respond well when caught and badly when run through, and this is not a case where waiting to see is a good strategy.

What Actually Helps

Reduce the load, don't stop entirely. Cut mileage substantially rather than dropping to nothing, and switch to softer surfaces. Cross-training keeps your fitness while the shins recover: cycling, swimming and the elliptical all load the legs far less than running.

Build the calves. Calf strength is the best-supported piece of the puzzle. Standing calf raises and seated calf raises, done slowly with control, three times a week. Progress gradually to single-leg versions. This is the work that stops it recurring, and it's the part most people skip once the pain fades.

Strengthen the hips too. As with runner's knee, hip control affects everything downstream. Glute bridges, clamshells and side-lying leg raises all help the lower leg by improving what's happening above it.

Check the shoes. Running shoes lose their cushioning long before they look worn out. If yours are past a few hundred miles, replacing them may solve more than any exercise will.

Getting Back to Running

Return gradually rather than picking up where you left off. Once you can walk briskly and hop on the affected leg without pain, start with short, easy runs on soft surfaces, ideally alternating running and walking. Increase by small increments week to week, and use the following morning as your guide: if the shins are worse the next day, you went too far, so drop back a step rather than pushing through.
